PLAYERS: To
be registered with Bonneville Youth Soccer League for Spring 2008
soccer, every player must have a signed (by parent or guardian)
registration form. Once you have been notified as to which team you
will be playing for, please remember that you MUST provide your coach
or team manager:
- Registration form signed by a parent or guardian (forms can be downloaded).
- Fee (see fee schedule)
- Proof-of-Age Document:
- Certified state-issued birth certificate
- Birth registration issued by appropriate government agency
- Board of Health records (notarized copy)
- Uniform service identification and privilege card (DD Form 1173, notarized copy)
- Current passport (notarized copy)
- Alien
registration card issued by U.S. Immigration and Naturalization Service
attesting to age or a certificate of an American born abroad issued by
the appropriate government agency
- Current driver's license

COACHES/MANAGERS:
To have a team registered with Bonneville Youth Soccer League for
Spring 2008, every coach or manager must turn the following paperwork
in to the Registrar:
- Roster
- Registration form for each player
- Fees
- Scholarships
- Coach/Assistant forms (Application, Code of Ethics, Employment/Volunteer Disclosure)
- Players' Proof-of-Age documents
- Play-up Consent Form
If a player comes to join the
team late, that is fine, just turn in his/her paperwork as soon as
possible to the BYSL Registrar ($5 late fee applies after March 1 -
IYSA only). Transferring players will cost $16. (Include
Add/Transfer/Release forms - IYSA only.)
