Registration Instructions for Bonneville Youth Soccer League

PLAYERS:

To be registered with Bonneville Youth Soccer League for Spring soccer, every player must have a signed (by parent or guardian) registration form. Once you have been notified as to which team you will be playing for, please remember that you MUST provide your coach or team manager:

• Registration form signed by a parent or guardian (forms can be downloaded).

• Fee

• Code of Conduct Forms – Player & Parent

• Proof-of-Age Document:

  • Certified state-issued birth certificate

  • Birth registration issued by appropriate government agency

  • Board of Health records (notarized copy)

  • Uniform service identification and privilege card (DD Form 1173, notarized copy)

  • Current passport (notarized copy)

  • Alien registration card issued by U.S. Immigration and Naturalization Service attesting to age or a certificate of an American born abroad issued by the appropriate government agency

  • Current driver's license

COACHES/MANAGERS:

To have a team registered with Bonneville Youth Soccer League for Spring of 2010, every coach or manager complete the following:

• Roster

• Registration form for each player – signed by guardian

• Code of Conduct Forms – Player & Parent

• Collect appropriate fees (turn in to BYSL treasurer)

• Scholarship requests to BYSL president

• Coach, Assistant Coach and Manager forms:

  • BYSL Coaching Request Form

  • Coach/Manager/Volunteer Add/Release Form

  • Code of conduct

• Players' Proof-of-Age documents

• Turn in all forms to BYSL Registrar

If a player comes to join the team late, that is fine, just turn in his/her paperwork as soon as possible to the BYSL Registrar. Include Add/Transfer/Release form. Transferring players will cost $16.
